If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We believe in the importance of quality dental care, and we strive to provide the best dental treatment possible. Also, we understand the financial limitations that influence your choice of care. We want to assure you of our flexible approach to financing.
We work with most insurance companies and always try to maximize your coverage through meticulous detailing of procedures and interaction with your insurer. We even fill out and submit your claim forms and we're available to answer any questions we can. However, if we do not receive payment from your insurance carrier within 90 days, you will be responsible for payment of your treatment fees and collection of your benefits directly from your insurance carrier.
Please remember, however, that you are responsible for the portion of your treatment not covered by insurance (should you have insurance). Because we, too, must balance our finances, we do ask that you pay your portion of the bill at the time of treatment (unless alternate arrangements have been made prior to the visit). If you qualify, we'll work with you to devise a method of payment that works for both of us. We accept checks, Care Credit and most major credit cards. (In those cases where balances remain after 30 days, an interest rate of 1.5% per mo will be assessed.)

For your convenience our office will submit charges for our services to your insurance carrier but we consider each patient responsible for his or her account. The service provided by a dentist is an agreement between the patient and our office. Insurance coverage is an agreement between the patient and the insurance company.
